Job summary

6 Degrees Search is seeking an experienced Home Health RN Case Manager to cover Fort Worth, TX and surrounding Tarrant County. This field-based role requires clinical excellence, strong assessment skills, and the ability to manage a patient caseload in the home setting.

Responsibilities include performing skilled nursing visits, developing plans of care with physicians and families, completing OASIS and documentation, and ensuring regulatory compliance while delivering high-quality care.

Qualifications

• Active Texas RN license in good standing.
• Recent Home Health RN Case Management experience required.
• Strong understanding of Medicare home health, OASIS, and plan-of-care management.
• Ability to work independently while maintaining strong communication with the clinical team.
• Valid driver’s license, reliable transportation, and ability to travel locally throughout Fort Worth / Tarrant County.
• EMR experience preferred.

Responsibilities

• Manage an assigned home health patient caseload in the field.
• Perform skilled nursing visits, assessments, evaluations, recertifications, resumptions of care, and discharges.
• Develop, update, and coordinate individualized plans of care in collaboration with physicians, patients, families, and the interdisciplinary care team.
• Complete accurate, timely clinical documentation, including OASIS and visit notes.
• Monitor patient progress, identify changes in condition, and communicate updates appropriately.
• Provide patient and caregiver education related to disease management, medications, safety, and plan-of-care goals.
• Coordinate care with therapy, social work, aides, physicians, and office/clinical leadership.
• Maintain compliance with home health regulatory, documentation, and quality standards.
