Responsibilities

• Serve as the primary relationship contact for an assigned portfolio of providers.
• Build strong provider relationships through visits, onboarding and support.
• Partner with providers to improve HEDIS and quality outcomes.
• Analyze provider quality, utilization and cost data to identify trends.
• Conduct provider performance reviews and develop action plans.
• Support provider engagement, retention, and network growth.
• Collaborate with Quality, Population Health, Credentialing, and Analytics teams.
• Monitor performance, document insights, and communicate risks to leadership.
• Promote high-quality, cost-effective care within value-based programs.
