Job summary

21st Century Home Health Services in Vacaville, CA is seeking an entry-level Field Physical Therapist to build core clinical skills in a home health setting. You will visit patients in their homes under the supervision of a Physical Therapist Case Manager and gain exposure to company operations and interdisciplinary collaboration.

Qualifications

• Current, unrestricted California Physical Therapist license.
• Current BLS/CPR certification.
• CA Driver’s license with clean driving record.
• Graduate of an accredited Physical Therapy program.
• One year of PT experience preferred; new grads may be considered.
• Home health experience is highly desirable.

Responsibilities

• Provide hands-on therapeutic interventions in home health.
• Assist with devices and home safety equipment management.
• Conduct detailed patient evaluations in the home setting.
• Implement plans of care under the supervision of the PT Case Manager.
• Document patient care within 24-48 hours.
• Educate patients and families on mobility and adaptive equipment.
• Collaborate with the PT Case Manager on treatments and updates.
• Participate in interdisciplinary services and report needs.
• Maintain privacy and dignity in care and documentation.
• Engage in ongoing professional development and trainings.
• Participate in on-call or weekend coverage as required.
