Qualifications

• Must have an ability to travel to outreach locations including medical centers, homeless shelters.
• Willingness and ability to travel across various locations regularly.
• Bachelor's Degree is preferred but experience can also be considered.

Responsabilités

• Provide outreach through distribution of materials and educational presentations.
• Assist program participants with accessing needed community services.
• Document all services in compliance with agency standards.
