Author Health in Medford, MA is seeking an Outreach Liaison to connect vulnerable patients with life-changing mental health care. You will conduct field outreach, educate patients and caregivers, coordinate appointments, and navigate complex health and social service systems.

This remote-first, field-based role requires 3+ years working with vulnerable populations, high emotional intelligence, resilience, and ability to drive coverage across regions.

Qualifications

• 3+ years of experience working with vulnerable populations in community settings.
• Experience in healthcare outreach, community engagement, or related field preferred.
• Crisis management experience a plus.

Responsabilités

• Field outreach to educate patients and caregivers about Author Health's services.
• Navigate complex healthcare and social service systems to guide patients through care options.
• Schedule and facilitate healthcare appointments and ensure transitions into ongoing care.
• Build and strengthen referral partner networks in the community.
• Document activities clearly using the CRM and EMR systems.
