Author Health, LLC is seeking an Outreach Liaison to connect vulnerable patients with mental health care in Medford, MA. The role combines autonomous field work with a strong support network, requiring empathy, resilience, and excellent communication.

The position offers a base salary plus performance-based incentives, with On-Target Earnings in the range of $60,500 to $66,000 annually. Travel within the Northeast may be required to support coverage needs.

Qualifications

• 3+ years of direct service experience with vulnerable populations in community settings.
• Experience in healthcare outreach, community engagement, or related field.
• Ability to navigate complex healthcare and social service systems.
• Experience meeting performance targets/quotas.

Responsabilités

• Conduct field outreach to educate patients and caregivers about Author Health's services.
• Guide patients through care options and coordinate healthcare appointments.
• Build and maintain a network of referral partners in the community.
• Document outreach activities clearly using CRM/EMR systems.
• Travel within required regions to support coverage periods.
