Job summary

Fortcom Health Services is seeking a full-time Field/Office Registered Nurse to join our Adult Family/Foster Care (AFC) team in Massachusetts. The Intake RN will focus on new member admissions, ensure clinical documentation is complete to start services, and conduct in-home supervisory visits every other month as required.

Responsibilities include on-site admission assessments, completing MDS-HC and related paperwork, developing Plans of Care with input from caregivers and the Program Director,

Qualifications

  • Associate’s or Bachelor’s degree in Nursing (BSN preferred).
  • Current Massachusetts RN license in good standing.
  • Ability to travel within assigned regions (mileage reimbursement provided).
  • Previous home healthcare or community-based nursing experience.
  • Experience with MDS-HC assessments and AFC or related programs is a plus.
  • Strong written and verbal communication skills.
  • Proficiency with Microsoft Office (Word, Excel, PowerPoint).
  • Bilingual in Spanish or Haitian Creole is a strong plus.

Responsibilities

  • Conduct on-site admission assessments for new AFC members, including the completion of the MDS-HC and other required clinical documentation.
  • Perform in-home supervisory visits to members' homes at least every two months and complete all required clinical documentation for those visits.
  • Ensure all initial paperwork is accurate and compliant with program requirements before services begin.
  • Develop and maintain individualized Plans of Care for each member, with input from the caregiver and Program Director.
  • Review and document physician summaries and coordinate medical clearances as needed.
  • Provide initial orientation and training to caregivers before care begins.
  • Monitor member health status and escalate changes to the appropriate physicians.
  • Complete timely and accurate nursing progress notes for each visit or encounter.
  • Assist with RN Monthly Visit Note Review as needed.
  • Complete Onsite Visit as needed.
  • Prepare MDSs as needed.
  • Assist in entering new referral/new member data on AFC including diagnosis and medications.
  • Participating in audits, quality management and program integrity process, Multidisciplinary team conferences and represents Agency in the community.
  • Perform periodic reviews of caregiver logs and ensure compliance with care standards.
  • Submit required reports, including semiannual health status updates to PCPs.
  • Assist with discharge planning when members transition out of the program.
  • Provide support and communication with caregivers, ensuring they are well-trained and confident in their role.

Position Summary

Fortcom Health Services is seeking a compassionate and highly organized Full-Time Field/Office Registered Nurse to join our Adult Family/Foster Care (AFC) team. This position is ideal for a nurse who thrives in a field-based setting and enjoys working independently while making a difference in the lives of medically eligible adults and their caregivers. This role will primarily focus on new member admissions and ensuring that all necessary clinical documentation is complete to start services.
