Qualifications

• Licensed OT in Michigan required.
• Experience delivering OT services in home care or field setting preferred.
• Strong interpersonal and communication skills.
• Proficiency with Kinnser software.

Responsibilities

• Perform comprehensive OT evaluations in clients' homes.
• Develop and implement individualized treatment plans.
• Provide hands-on interventions to improve independence and quality of life.
• Document progress notes and treatment plans timely.
• Communicate with clients, caregivers, and healthcare professionals to coordinate care.
• Maintain compliance with policies, standards and regulations.
