In this role, you will deliver personalized care in the field, helping individuals improve their daily living skills and achieve their personal goals. You will work collaboratively with a multidisciplinary team to develop and implement effective treatment plans tailored to each client’s unique needs.

Responsibilities

• Perform comprehensive occupational therapy evaluations and assessments in clients' homes.
• Develop and implement individualized treatment plans to improve clients’ functional abilities.
• Provide hands-on therapeutic interventions to enhance clients’ independence and quality of life.
• Document all client interactions, progress notes, and treatment plans accurately and timely.
• Communicate regularly with clients, caregivers, and healthcare professionals to coordinate care.
• Maintain compliance with company policies, professional standards, and industry regulations.

Requirements

• Licensed Occupational Therapist (OT) in the state of Michigan.
• Proven experience providing occupational therapy services in a home care or field setting preferred.
• Strong interpersonal and communication skills.
• Experience with Kinnser
• Ability to work independently and manage a flexible schedule.
• Valid driver’s license and reliable transportation.
• Commitment to delivering compassionate and patient-centered care.
