Job summary

Executive Home Care is seeking per-diem Registered Nurses to admit clients, build plans of care, and safeguard home-based care through a structured monitoring cycle.

You will conduct initial admissions, 6-month reassessments, and regular check-ins, while supervising aides and LPNs. Documentation in WellSky is required, with visits scheduled around your availability and some weekends/holidays included.

Qualifications

• RN licensed to practice in New Jersey.
• Experience with admissions and care planning.
• Familiarity with WellSky EHR.

• 30-day client check-in calls and 60-day case-monitoring visits, completed on schedule.
• Supervise and support home health aides and LPNs in the field.
• Non-Skilled nursing visits when assigned and accurate, timely documentation in WellSky.
• Observe and report changes in condition promptly and elevate safety concerns per protocol.

WHAT YOU'LL DO

Paid on regular payroll once documentation is complete and on time. This is a per diem position: assignments are scheduled as needed some weekends and holidays. Build visits around your schedule.
