Job summary

Akido Labs, Inc. seeks a Nurse Practitioner to join the Street Medicine team in Kern County, delivering frontline care to unhoused and other vulnerable populations. The role combines in-field clinical duties with AI-driven tools to enhance accuracy and efficiency.

You will work under physician supervision, travel up to 75% of time, and collaborate with a multidisciplinary team to design and implement care plans aimed at stability and better health outcomes.

Qualifications

• Active, unrestricted California NP Certificate by hire date.
• Nurse Practitioner Furnishing Number (California) required.
• Registered Nurse License (California) required.
• Ability to travel up to 75% of work time.
• In-person field-based care experience is valued.

Responsibilities

• Provide frontline care to patients experiencing homelessness, behavioral health challenges, SMI/SUD, and complex chronic conditions in the field.
• Manage diverse patient panels with acute, chronic, and preventive care.
• Conduct assessments, histories, exams, order/interpret diagnostics, and prescribe under physician supervision.
• Collaborate with a multidisciplinary team to coordinate care and address complex cases.
• Design and implement care plans to promote stability and improved health outcomes.
• Document all encounters for continuity of care.
