Job summary

ConcertoCare’s Field Nurse (LPN) role involves delivering home-based, patient-centered care for seniors and adults with complex needs. You will work with an interdisciplinary team to provide wrap-around services during home visits, performing clinical tasks and coordinating care to improve outcomes.

Frontline workers must be fully vaccinated; compounds include a competitive salary, sign-on bonus eligibility, and comprehensive benefits. Travel to patient locations is required.

• Engage hard-to-reach patients and bring vital care to them at home.
• Ensure continuity of care by providing coverage for team members when needed.

Requirements

• Licensed Practical Nurse (LPN) licensure in the state of employment
• Ability to conduct independent home visits
• Experience providing care for the whole person, including caring for adults and seniors with high medical and social complexity
• Current CPR and BLS certifications required
• Intermediate to advanced computer and software skills, including Patient Health Exchange Platforms, Electronic Health Records, and Outlook
• Ability and means to travel as needed in a timely manner to locations that may have limited access to public transportation; proof of liability and property damage insurance on vehicle used is required.
• DRIVER REQUIREMENTS: Licensed for a minimum of 5 years. No vehicle-related suspensions, DUI, reckless driving, leaving the scene of an accident, vehicular manslaughter, felony convictions, or 3 or more moving violations within the last 5 years. No more than 2 moving violations or 1 at-fault accident in the last 3 years.

ConcertoCare requires all “frontline workers” to be fully vaccinated and to provide records for validation. Medical or religious exemptions will be considered contingent upon the review of appropriate documentation.

Base Salary/ Wage Range $35.00 to $38.00/hour with annual bonus. Compensation for the role is commensurate with the candidate’s qualifications, skills, competencies, and experience and may fall outside of the range shown.
