Job summary

ConcertoCare is seeking a Field Nurse to provide licensed practical nursing in patients' homes across Kentucky. This full-time role combines compassionate, tech-enabled care with a value-based model.

Qualifications

• Licensed Practical Nurse licensure in the employment state.
• Ability to conduct independent home visits.
• Experience caring for adults and seniors with high medical/social complexity.
• Current CPR and BLS certifications.
• Ability and means to travel to locations with limited public transport; vehicle insurance proof required.
• Driver requirements: clean record per the stated policy.

Responsibilities

• Develop and implement patient-centered clinical plans with the care team.
• Deliver wrap-around services through home visits and collaborative care.
• Perform chronic condition care, wound care, phlebotomy, medication review, and more.
• Engage hard-to-reach patients with essential at-home care.
• Ensure continuity of care by covering for team members when needed.
• Drive better patient outcomes and satisfaction through interdisciplinary care.

Your Impact

Requirements

• Licensed Practical Nurse (LPN) licensure in the state of employment
• Ability to conduct independent home visits
• Experience providing care for the whole person, including caring for adults and seniors with high medical and social complexity
• Current CPR and BLS certifications required
• Intermediate to advanced computer and software skills, including Patient Health Exchange Platforms, Electronic Health Records, and Outlook
• Ability and means to travel as needed in a timely manner to locations that may have limited access to public transportation; proof of liability and property damage insurance on vehicle used is required.
• DRIVER REQUIREMENTS: Licensed for a minimum of 5 years. No vehicle‑related suspensions, DUI, reckless driving, leaving the scene of an accident, vehicular manslaughter, felony convictions, or 3 or more moving violations within the last 5 years. No more than 2 moving violations or 1 at‑fault accident in the last 3 years.

ConcertoCare requires all “frontline workers” to be fully vaccinated and to provide records for validation. Medical or religious exemptions will be considered contingent upon the review of appropriate documentation.

Salary

$26-$31/hr

Base Salary/ Wage Range $26.00 to $31.00/hour with annual bonus. Compensation for the role is commensurate with the candidate’s qualifications, skills, competencies, and experience and may fall outside of the range shown.
