Job summary

Paradigm is seeking a Field Nurse Case Manager to cover the South Bay area in San Francisco, CA and surrounding areas. This is a home-based role with travel to doctors’ offices, hospitals, and other locations.

The Case Manager handles medical case management for work-related injuries, including assessment, planning, coordination, implementation, and evaluation with insurance carriers and providers. The position requires an active California RN license and preferred CCM/CRRN/CDMS/CRC

Qualifications

• RN license in California is required.
• Experience in workers' compensation case management is preferred.
• Certifications CCM, CRRN, CDMS, CRC strongly preferred; must be obtained within two years of hire.

Responsibilities

• Medical case management of work-related injuries, including assessment, planning, coordination, implementation and evaluation.
• Coordinate with insurance carriers, medical providers, employers and employees, reporting results to employers and insurers.
