Job summary

Paradigm is seeking a Field Nurse Case Manager to cover Dayton, OH and surrounding communities, including Cincinnati and Columbus. This is a hybrid position requiring residency in the Dayton area due to 80% local travel for in-person patient visits.

The role focuses on medical case management of work-related injuries, coordinating care with insurers, providers, and employers while pursuing catastrophic case readiness.

Qualifications

• Current, unencumbered RN license in OH.
• Experience in case management of workers' compensation cases preferred.
• Willingness to learn and take catastrophic cases.
• Certifications such as CCM, CRRN, CDMS, CRC preferred; required within two years of hire.
• Strong organization, communication, and technology skills; ability to work autonomously.

Responsibilities

• Manage medical case work-related injuries: assessment, planning, coordination, implementation and evaluation.
• Collaborate with insurance carriers, medical providers, attorneys, employers and employees; monitor progress and report results.
