trustedcare medical group is seeking a Neurology PA-C or Nurse Practitioner to serve as a field-based provider in SNFs and ALFs in Arizona. You will deliver on-site neurology evaluations and manage complex post-acute and long-term care patients.

Work independently in the field while collaborating with supervising neurologists and facility teams to ensure patient-centered care, adherence to state laws, and quality outcomes. Travel required; competitive compensation and benefits offered.

Qualificações

• Graduate of a PA-C or NP program and licensed to practice in AZ.
• Board certification by NCCPA or relevant nursing certifying body.
• Authorized to prescribe controlled substances under AZ scope.
• DEA registration in AZ for prescribing CS.

Responsabilidades

• Perform comprehensive neurological assessments in SNFs/ALFs.
• Diagnose and manage post-acute and long-term neurological conditions.
• Order and interpret diagnostic studies; coordinate with labs and hospitals.
• Prescribe medications within licensure; develop individualized care plans.
• Educate patients, families, and staff on conditions and regimens.
• Support transitions from hospital to SNF/ALF with continuity of care.
• Collaborate with facility teams and supervising neurologists.
