Qualifications

Texas LVN license required.

Reliable transportation and valid driver’s license.
CPR certification preferred.

Responsibilities

Provide skilled nursing care to patients in their homes per physician orders and care plans.
Monitor patient health status and report changes to supervising nurse or physician.
Administer medications, injections, and treatments as prescribed.
Assist patients with activities of daily living (ADLs).
Document patient care accurately and timely.
Educate patients and families on health management and medications.
