Uniting Holding seeks a licensed practical nurse (LPN) or licensed vocational nurse (LVN) to provide safe, quality home health services in accordance with agency policies. The role includes preparing clinical notes, assisting physicians and RNs with procedures, and teaching patients self-care techniques.

A valid state license and health clearance are required. Prefer one year of nursing experience, especially in community/home health or medical/surgical settings.

Formación

• Must be licensed as LPN or LVN in the State.
• Prefer one year nursing experience; community/home health or medical/surgical preferred.
• Knowledge of communicating with individuals across age groups.
• Complies with accepted professional standards and practice.

Responsabilidades

• Furnishes home health services per agency policies and prepares clinical and progress notes.
• Assists physician and RN in procedures; prepares equipment and materials observing aseptic technique.
• Provides patient and family education on self-care.
• Implements plan of care initiated by the RN Case Manager in collaboration with the physician.
• Maintains patient documentation and communicates condition changes to the RN Case Manager.
