If you are located within the Paris, TX market, you will have the flexibility to work remotely* as you take on some tough challenges.

This is a field-based role with around 75% travel in the field and a home-based office.

Primary Responsibilities:
  1. Assess, plan, and implement care strategies individualized for each patient, aimed at the most appropriate, least restrictive level of care.
  2. Identify and initiate referrals for social service programs, including financial, psychosocial, community, and state supportive services.
  3. Manage the care plan throughout the continuum of care as a single point of contact. Communicate with all stakeholders to ensure quality, coordinated care and services are provided expeditiously to all members.
  4. Advocate for patients and families as needed to ensure their needs and choices are fully represented and supported by the healthcare team.

Required Qualifications:
  • Current unrestricted RN license in Texas or a compact RN license
  • 2+ years of experience in community health or a healthcare role
  • Intermediate proficiency with Microsoft Office (Word, Excel, Outlook)
  • Reliable transportation and ability to travel within the assigned region to visit members in various settings
  • High-speed internet at residence
  • Valid driver’s license
Preferred Qualifications:
  • 1+ years of experience with long-term care services, Medicaid, or Medicare
  • Ability to create, edit, and send documents, spreadsheets, and emails
  • Knowledge of federal and state regulations regarding integrated settings
  • Reside within a reasonable distance of the assigned duties

The salary range for this role is $28.61 to $56.06 per hour, based on full-time employment, local labor markets, experience, and certifications. Benefits include a comprehensive package, incentives, and stock purchase options. Details are available upon request.
