Job summary

Blue Shield of California Promise Health Plan seeks an Experienced Community Health Advocate to report to the Sr. Director, Community Programs. You will conduct field-based outreach with members through home visits, in-person support at medical appointments, and comprehensive assessments of social drivers of health (SDOH).

Responsibilities

• Conduct field-based outreach with Blue Shield of California Promise Health Plan members through regular home visits and in-person support at medical appointments.
• Assess social drivers of health (SDOH) and identify resource gaps in medical and community services.
• Connect members to appropriate health and social services.
• Engage members in community-based settings and travel throughout Los Angeles County.
