This position oversees all contracted care coordination (CC) activities for the CHOICES program, as defined in the contract, TennCare policy, or protocol. It is a field-based role requiring travel to assess, create care plans, and authorize LTSS services within prescribed timeframes. The team manages LTSS members in Davidson and Williamson County daily.

If you reside in Davidson or Williamson County, TN, you will have the flexibility to work remotely* while taking on these responsibilities.

Primary Responsibilities:

1. Travel to assess and create plans of care
2. Authorize LTSS services within prescribed timeframes
3. Manage LTSS members in Davidson and Williamson County on a daily basis

Required Qualifications:

• Current, unrestricted LBSW, LMSW, LCSW licensure, or LPN licensure
• At least 1 year of clinical or case management experience
• At least 1 year of experience in long-term care, home health, hospice, public health, or assisted living
• Valid driver’s license and access to reliable transportation for field visits

Preferred Qualifications:

• Medicare/Medicaid experience
• Experience with electronic charting
• Background in geriatrics or special needs populations
• Experience in behavioral health

The hourly wage range for this role is $28.61 to $56.06, based on full-time employment, local labor markets, education, experience, and certifications.
