This role oversees all contracted care coordination (CC) activities for the CHOICES program, as defined in the contract, TennCare policy, or protocol. It is a field-based position requiring travel to assess, create care plans, and authorize LTSS services within specified timeframes. The team manages daily operations for LTSS members in Davidson and Williamson County.
If you reside in Davidson or Williamson County, TN, you will have the flexibility to work remotely, while managing field visits and assessments.
This role offers a pay range of $28.61 to $56.06 per hour, based on full-time employment, experience, and other factors. Benefits include comprehensive packages, incentive programs, stock purchase options, and 401k contributions. All remote employees must adhere to UnitedHealth Group’s Telecommuter Policy.
