Job summary

Oklahoma Department of Human Services in Stillwater, Oklahoma seeks a Case Manager I/II to support individuals with intellectual and/or developmental disabilities in home and community settings. You will assess needs, connect to resources, and monitor progress under supervision.

Minimum qualifications include a bachelor’s degree in a human services field with 1 year of relevant experience, or a permanent Oklahoma nursing license with 1 year of related experience. A training program is provided.

Qualifications

Bachelor’s degree in a human services field with 1 year of relevant experience.
Or permanent Oklahoma nursing license with 1 year of relevant experience.
Level II requires 2 years of related experience.

Responsibilities

Under supervision, provide services to individuals with intellectual and/or developmental disabilities in home or community settings; assess needs and coordinate resources; monitor progress.
Ensure compliance with Federal Home and Community Based Waiver regulations, state, and policy guidelines.
