Job summary

National Youth Advocate Program Tucker, GA is seeking a contract Community Support Specialist to join our OPMH program. You will work with youth and families, providing counseling, advocacy, and community-based support to help them live safely in the community.

Under supervision, you will travel to serve youth, document activities, participate in supervision, and collaborate with foster caregivers and partners to improve outcomes.

Qualifications

• Bachelor's or Master’s in Social Work or related Human Services field.
• 2 years direct service experience with youth and families preferred.
• Valid driver’s license, reliable transportation and auto insurance.
• Willingness to work flexible, non-traditional hours.
• Flexible and responsive to youth, caregivers, system partners, and internal customers.
• Objective, professional, and respectful with diverse groups.
• Proficient with computers and common software (Word, Excel, etc.).

Responsibilities

• Supports NYAP mission, vision, and values.
• Provides individual, group, and family mental health services and case management.
• Travels daily to deliver community-based services.
• Completes required documentation promptly.
• Completes required training promptly.
• Meets productivity thresholds.
• Coordinates and monitors youth and family services across needed supports.
• Reviews and updates youth plans for progress.
• Maintains strong relationships with foster parents and families.
• Assists in family reunification or permanency planning.
• Provides safety assessment, crisis response, and behavioral stabilization.
• Participates in CQI activities and weekly supervision.

Education
