Job summary

Child & Family Focus, Inc. in Montgomery County, PA is seeking a dedicated Case Manager to provide supportive care to identified children with behavioral health diagnoses and their families.

You will help families navigate services, complete assessments, and coordinate resources, with face-to-face delivery emphasized. Responsibilities include developing treatment plans, maintaining caseloads, and collaborating with schools, providers, and community agencies.

Qualifications

• 12 credits in coursework related to Human Services, Psychology or related field.
• One or more years of direct clinical experience with children, youth and families.
• Ability to manage a varied Monday–Friday schedule 8:30am–8:30pm, with 3 evenings per week.

Responsibilities

• Provide case management and service planning for identified clients.
• Assure environmental matrices and written service plans.
• Conduct home, school, hospital, and community visits daily; travel as needed.
• Coordinate services with other agency providers and resources.
• Provide on-call crisis support in rotation with staff.
