Job summary

Family & Children’s Agency (FCA) in Norwalk, CT seeks a Street Outreach Case Manager to provide immediate access to coordinated entry and essential services for unsheltered individuals. Qualifications

• BA in related field preferred.
• Bilingual/Spanish speaking required.
• Experience with homeless population and mental health/substance abuse awareness.
• Reliable transportation and a valid Driver’s License.
• Awareness of cultural and socioeconomic characteristics of clients.

Responsibilities

• Conduct regular and targeted street outreach to locations frequented by unsheltered individuals.
• Engage with clients using sensitive strategies and build trust.
• Connect with people and inform them about housing and services.
• Provide crisis counseling and immediate support.
• Distribute meals, blankets, clothing, and toiletries.
• Publicize CAN/CoC services and coordinate entry appointments.
• Refer clients to CAN Navigators for eligibility guidance.
• Share information about homeless and mainstream programs.
