Job summary

Health Link is seeking a motivated Account Manager to join our Community Relations team, supporting growth across Santa Clara County. This field-based role focuses on developing and maintaining referral relationships with healthcare providers to expand Health Link’s home health services.

You will work closely with the Director of Business Development, conduct in-person outreach, and educate partners on our clinical programs and capabilities.

Qualifications

• Sales-oriented mindset with motivation to develop new business.
• Comfortable conducting in-person outreach and developing new professional relationships.
• Self-motivated and comfortable working independently in the field.

Responsibilities

• Develop and maintain referral relationships throughout Santa Clara County.
• Build relationships with hospitals, SNFs, physicians, medical groups, clinics, senior living communities, and other healthcare organizations.
• Conduct in-person outreach, account visits, meetings, and presentations.
• Educate partners on Health Link’s home health services and programs.
• Meet referral, admission, and account growth goals.
