Job summary

Ultipro in Saginaw, MI is seeking a Social Worker to coordinate MIHP services, perform home visits, and support families through enrollment and engagement. Responsibilities

• Coordinates and monitors all MIHP services for assigned beneficiaries; conducts home and office visits and completes assessments.
• Participates in interdisciplinary care planning to develop Plans of Care and supports patient self-management.
• Accepts referrals and communicates with care team to address ongoing needs.
• Connects patients with medical and community resources and documents referrals.
• Maintains Medicaid/MIHP documentation; ensures timely and accurate records and HIPAA compliance.
• Participates in QA, QI and certification activities and improves participant engagement and service delivery.
• Conducts outreach and health promotion activities for MIHP and supports recruitment.
