Adaptive Home Health is seeking a Speech-Language Pathologist (SLP) to provide in-home evaluations and skilled visits across a field-based territory in Nevada. You will own outcomes, develop plans of care, and coordinate with the care team to optimize communication and swallowing safety for home-bound patients.

You will work full-time, Monday–Friday with occasional weekends, earning compensation tailored to productivity (65–75 per point, 28 points per week).

Qualifications

• Active Nevada SLP license in good standing.
• Strong clinical reasoning with ability to work independently in the field.
• Comfort identifying red flags and escalating changes to RN/MD.
• Strong caregiver education skills (teach-back, carryover planning).
• Strong communication and documentation.
• Reliable follow-through on orders, frequencies, and patient goals.
• Current driver’s license and comfort with daily travel in assigned territory.

Responsabilités

• Own SLP outcomes, plan, progression and functional results.
• Complete comprehensive in-home SLP evaluations and skilled visits.
• Assess and treat Aphasia, cognitive-communication issues, and dysphagia as ordered.
• Build and execute a clear plan of care with measurable goals.
• Coordinate with multidisciplinary team and communicate changes.
• Coach patients and caregivers with practical strategies for daily routines.
• Document promptly, defensively and thoroughly.
