Job summary

Adaptive Home Health is seeking a Speech-Language Pathologist (SLP) to provide evidence-based assessments and skilled visits in patients' homes across Texas. You will evaluate communication, cognition, and swallowing safety, while coaching caregivers for carryover between visits.

You’ll operate independently with strong clinical reasoning, maintain thorough documentation in HCHB, and participate in a schedule that balances field travel with patient needs.

Qualifications

• Active Texas SLP license (in good standing).
• Strong clinical reasoning and ability to operate independently in the field.
• Comfort identifying red flags and escalating changes in condition to RN/MD.
• Strong caregiver education skills (teach‑back, carryover planning).
• Strong communication and documentation.
• Reliable follow-through on orders, frequencies, and patient goals.
• Current driver’s license and comfort with daily travel across assigned territory.

Responsibilities

• Own outcomes, not just visits: accountable for the patient’s SLP plan, progression, and functional outcomes.
• Complete comprehensive in-home SLP evaluations and deliver skilled SLP visits.
• Assess and treat: aphasia, cognitive-communication impairment, and dysphagia screening/education.
• Build and execute a clear plan of care with measurable goals.
• Drive multidisciplinary coordination and communicate changes in condition to the care team.
• Coach patients/caregivers with practical strategies and routines.
• Document promptly and thoroughly in Homecare Homebase (HCHB).
