Job summary

All Metro Health Care is hiring an RN Clinical Supervisor to oversee home care patients in Manhattan and Staten Island. The role combines clinical oversight with field visits to patients' homes and caregiver support in a community-based setting.

The ideal candidate holds an active NY RN license, CPR certification, and at least 1 year of relevant RN experience, with strong assessment and communication skills for coordinating care with families and the team.

Qualifications

• Active New York State RN license.
• Current CPR certification.
• At least 1 year of RN experience in a skilled nursing or clinical care setting.
• Strong clinical assessment, communication, and organizational skills.
• Ability to work independently and travel to patients' homes throughout the territory.

Responsibilities

• Conduct initial and ongoing nursing assessments in patients' homes.
• Perform supervisory visits to support caregivers and ensure quality care.
• Evaluate patient needs and collaborate on individualized care plans.
• Complete follow-up visits after hospitalizations, falls, incidents, or changes in condition.
• Provide clinical guidance, education, and support to caregivers and patients.
• Participate in TBI and NHTD waiver care coordination meetings.
• Maintain accurate clinical documentation and ensure compliance with applicable regulations.
• Collaborate with branch leadership and provide coverage for the Clinical Manager as needed.
