Care Finders Total Care | A Modivcare Personal Care Service in Union County, NJ is seeking a Full-Time RN Clinical Supervisor to oversee home health care teams and ensure high-quality patient outcomes. This field-based role offers independent work and close collaboration with physicians to tailor services to each patient’s needs.

Qualifications

• Active New Jersey Registered Nurse (RN) license or appropriate authorization to practice nursing in the state.
• Current CPR certification.
• Strong clinical assessment, communication, and patient education skills.
• Ability to work independently in a field-based home care environment.
• Ability to travel throughout Union County for assigned patient visits.

Responsibilities

• Conduct nursing assessments and supervisory visits in patients' homes.
• Supervise, evaluate, and provide clinical guidance to Certified Home Health Aides (CHHAs).
• Develop, review, and update individualized plans of care.
• Monitor patients' conditions and communicate changes to the appropriate care team members.
• Provide clinical orientation, education, and training to caregivers.
• Collaborate with physicians and the care team to coordinate patient services.
• Maintain accurate and timely clinical documentation in accordance with company policies and applicable regulations.
