Job summary

Adaptive Home Health is seeking a full-time Registered Nurse Case Manager to deliver high-acuity home health services across Texas. You will own patient assessments, manage visits, and coordinate care to drive positive outcomes for patients in their homes.

The role offers a productivity-based pay model at 58.80 per point, with an expectation of 30 points per week; mileage is reimbursed in addition to productivity compensation.

Qualifications

• Current Texas RN license.
• 3+ years of clinical nursing experience.
• Comfort with wound care and infusions.
• CPR (BLS) required.
• Field-based work with Texas driver’s license.
• Strong time management and organization in a high-autonomy environment.
• Clear, professional communication with patients, families, and providers.

Responsibilities

• Own patient assessments and skilled visit documentation including OASIS.
• Deliver high-acuity home health care focusing on wound care and infusion therapy.
• Build and maintain a plan of care through communication with physicians, care team, patients and families.
• Serve as the patient advocate and ensure continuity of care across disciplines.
• Drive case management responsibilities including care coordination and follow-ups.
• Coordinate services to ensure the plan of care is executed.
• Administer medications and provide patient and caregiver teaching.
• Coordinate with internal teams to deliver care reliably and safely.
• Provide supervision to LVNs and HHAs when applicable.
• Participate in case conferences and in-services to align on quality and outcomes.
