Job summary

Adaptive Home Health is seeking a Registered Nurse Case Manager to lead skilled visits in a field-based role. You’ll manage assessments, documentation, wound care, and infusion therapy while coordinating with physicians and the care team to maintain patient safety and progress.

You will work in a high-autonomy environment with a productivity model paid per point and an expectation of 30 points per week. Mileage is reimbursed and scheduling supports field-based operations.

Qualifications

• Active Texas RN license
• 3+ years clinical nursing experience
• Comfort with wound care and infusions
• CPR (BLS)
• Current Texas driver’s license and field-based work readiness
• Strong time management and organization in a high-autonomy environment
• Clear, professional communication with patients, families, and providers

Responsibilities

• Own patient assessments and skilled visit documentation including OASIS.
• Deliver high-acuity home health care with wound care and infusion therapy.
• Build and maintain a clear plan of care through consistent communication with physicians, care team members, patients, and families.
• Serve as the patient's advocate and ensure continuity of care across disciplines.
• Drive case management responsibilities including care coordination, timely follow-ups, and barrier resolution to keep patients progressing toward goals.
• Coordinate with internal clinical and operational support teams to ensure safe, reliable delivery of care.
• Provide supervision and guidance to LVNs and HHAs when applicable.
• Participate in case conferences and in-services to align on quality, safety, and clinical outcomes.
