Adaptive Home Health is hiring an RN Case Manager to deliver high-acuity home health care, focusing on wound care and infusions. Field-based role with autonomy, supervising LVNs/HHAs and coordinating with physicians and care teams to maintain strong patient outcomes.

You will document visits, manage care plans, and advocate for patients while contributing to our mission of expanding access to high-quality home health care.

Qualifications

• Active Texas RN license.
• 3+ years of clinical nursing experience.
• Experience with wound care and infusions.
• CPR (BLS) certification.
• Current Texas driver's license and field-based work willingness.
• Strong time management and professional communication.
• Comfort with autonomous, high-autonomy environment.

Responsibilities

• Own patient assessments and skilled visit documentation including OASIS.
• Deliver high-acuity home health care with focus on wound care and infusion therapy.
• Build and maintain a clear plan of care through communication with physicians and families.
• Advocate for patients and ensure continuity of care across disciplines.
• Drive case management responsibilities including care coordination and follow-ups.
• Coordinate services to ensure the plan of care is executed.
• Administer medications and provide patient/caregiver teaching.
• Coordinate with clinical and operational teams to ensure safe, reliable care.
• Provide supervision to LVNs and HHAs when applicable.
• Participate in case conferences and in-services to align on outcomes.
