Adaptive Home Health, LLC is seeking a full-time RN Case Manager. This field-based role delivers high-acuity in-home care, focusing on wound care and infusion therapy, and builds comprehensive care plans through collaboration with physicians, care teams, patients and families.

You will drive case management activities, coordinate services, and supervise LVNs/HHAs when needed, with strong emphasis on autonomy, patient outcomes, and safety in a dynamic home-health setting.

Qualifications

• Current Texas RN license.
• 3+ years of clinical nursing experience.
• Comfort with wound care and infusions.
• CPR (BLS) certification.
• Texas driver’s license and willingness to field-based work.
• Strong time management and organization in a high-autonomy environment.
• Clear, professional communication with patients, families, and providers.

Responsibilities

• Own patient assessments and skilled visit documentation including OASIS.
• Deliver high-acuity home health care, with focus on wound care and infusion therapy.
• Build and maintain a clear plan of care through consistent communication with physicians, care team members, patients, and families.
• Serve as the patient advocate and ensure continuity of care across disciplines.
• Drive case management responsibilities including care coordination, timely follow-ups, and barrier resolution to keep patients progressing toward goals.
• Coordinate services to ensure the plan of care is executed.
• Administer medications and treatments as ordered, and provide patient and caregiver teaching.
• Coordinate with internal clinical and operational support teams for reliable and safe care.
• Provide supervision to LVNs and HHAs when applicable.
• Participate in case conferences and in-services to align on quality, safety, and clinical outcomes.
