Job summary
T O N E Home Health Services is seeking a Full-Time RN Case Manager for the Farmington Hills area. This position requires strong assessment and communication skills to develop individualized care plans and deliver nursing interventions in patients' homes.
The RN will coordinate patient care with physicians and team members, ensuring compliance with health standards while enjoying flexible scheduling and competitive per-visit compensation. A Michigan RN license and nursing experience are required.
Qualifications
- Minimum of 2 years of nursing experience; home health experience preferred.
- Reliable transportation and valid auto insurance required.
Responsibilities
- Conduct comprehensive nursing assessments.
- Develop individualized plans of care.
- Deliver skilled nursing interventions in the home.
- Educate patients and families on condition management and medications.
- Coordinate with physicians and interdisciplinary team members.
