Job summary

T.O.N.E. Home Health Services in Michigan is seeking an experienced RN Case Manager to provide compassionate, in-home care across West Oakland County and the listed towns, including Milford, South Lyon, Highland, White Lake, and Novi.

You will assess needs and coordinate skilled nursing support in patients’ homes. This role emphasizes strong clinical judgment, collaboration with an interdisciplinary team, and precise documentation in WellSky / Kinnser, while maintaining compliance with state,

Qualifications

• Graduated from an accredited school of nursing.
• Hold an active Michigan RN license.
• Have at least 2 years of nursing experience.
• Home health experience preferred.
• Are compassionate, resourceful, and highly organized.
• Have strong clinical judgment and communication skills.
• Have reliable transportation, auto insurance, and a valid driver’s license.

Responsibilities

• Conduct comprehensive nursing assessments.
• Develop individualized plans of care.
• Deliver skilled nursing interventions in the home.
• Educate patients and families on disease management and medications.
• Coordinate with physicians, therapists, aides, and interdisciplinary team members.
• Complete accurate and timely documentation in WellSky / Kinnser.
• Utilize Home Health Notify for real-time team communication.
• Ensure compliance with state, federal, CHAP, and Joint Commission standards.

Skills

Active Michigan RN license

2+ years nursing experience

Home health experience preferred
