Qualifications

• 2+ years in a quota-carrying, field-based sales or business development role.
• Experience prospecting into a defined target list of external partners.
• Ability to execute a structured sales or engagement process.
• Proven quota attainment or activity metrics.
• CRM or pipeline-tracking system experience.

Responsibilities

• Own and grow a portfolio of referral relationships with hospitals, SNFs, and community partners.
• Execute prospecting, cold outreach, and tailored solutions for partners.
• Mobilise referral partners by aligning services to client needs and care gaps.
• Maintain CRM hygiene, track activity and outcomes, and drive field outreach.
• Spend ~75% of time in the community delivering field-based engagement.
