Adaptive Home Health, LLC is seeking a Physical Therapist (PT) for a full-time field-based role in Las Vegas, NV. The position offers a flexible schedule and focuses on bringing high-quality home health care to patients.

You will work closely with patients on their therapy plans directly in their homes. The role involves thorough assessments, setting individual goals, and ensuring patient independence in daily activities.

• Role: Physical Therapist (PT)

• Schedule: Full-time Monday–Friday + occasional weekends (as needed)

• Location: Field Based

• Compensation (Productivity Points): $80 - $90 / point | 28 points per week

Comp Breakdown:

SOC: $200-$225

Evals: $120-$135

Recert: $140-$157.50

ROC: $160-$180

OASIS DC: $100-$112.50

Routine: $80-$90

Adaptive Home Health is building a higher-acuity, patient-centered, skilled home health model across Nevada. Our ultimate mission is to dramatically improve patient access to home health care.

The PT role is the front line of that mission: you restore function in the home, build safe routines, reduce avoidable decline, and help patients regain independence in ADLs/IADLs.

We build technology to better support our field clinicians faster and more supported. Ultimately, our clinicians are the customers of our engineering team. Our charting product is designed to reduce documentation burden and keep visits moving, and our scheduling product (launching soon) will make it easier to coordinate visits, reduce last-minute changes, and protect clinician time.

If you have strong clinical judgment and you like owning outcomes, this role gives you autonomy, meaningful patient impact, and a support team built to remove administrative friction.

What you will do:

• Own the caseload as a case manager: you’re accountable for the patient’s therapy plan, progression, and outcomes — not just individual visits.

• Complete comprehensive in-home assessments and deliver skilled PT visits.

• Build and execute a clear plan of care: gait/balance, transfers, strengthening, functional mobility, home safety, fall prevention.

• Set measurable goals, track objective progress, and adjust treatment quickly when patients stall.

• Drive multidisciplinary coordination: communicate changes in condition, align on orders, and keep the care team moving.

• Manage treatment and outcomes directly, including PTA supervision where appropriate.

• Coach patients and caregivers with practical home programs that actually get done.

• Document promptly and thoroughly — defensible, efficient, and consistent.

Must-haves

• Active Nevada PT license (in good standing)

• Strong clinical judgment and comfort working independently in the field

• Comfortable escalating changes in condition to RN/MD and documenting rationale clearly

• Strong communication and documentation

• Reliable follow-through on orders, frequencies, and patient goals

• Current driver’s license and comfort with daily travel across assigned territory

• Able to carry/transport basic therapy equipment as needed

• Competency in core home health PT scope

  • Safety-first mobility work: transfers, gait training, balance/fall prevention, assistive device recommendations

  • Strengthening, functional mobility, and home safety assessment/modification education

  • Patient/caregiver coaching with practical home programs

• Prior home health experience

• Deeper clinical experience in high-signal PT populations
