Healthfirst is seeking a Peer Support Specialist to use lived experience to assist members with mental health and/or substance use challenges. You will engage with members in field-based and telephonic settings, linking them to resources and coordinating with the care team to support recovery and person-centered care.

You will document encounters, participate in assessments, and collaborate with the Healthfirst Care Management Team to ensure effective care planning.

Qualificações

• High school diploma or equivalent.
• Certification or Provisional Certification as Peer Specialist or Recovery Peer Advocate.
• Demonstrated ability to manage a caseload and work effectively within a team, in a fast-paced environment.
• Proficient computer skills, including Outlook, Word and Excel.
• Ability to travel around downstate New York including the 5 boroughs, Long Island, Orange County, and Westchester.

Responsabilidades

• Manages designated caseload of members who could benefit from Peer Support services, care coordination, and health navigation.
• Schedules and conducts multiple field-based visits per day (clustered by geographic areas).
• Completes assessments and questionnaires to identify barriers and opportunities for intervention.
• Works telephonically to conduct outreach calls and handle benefit questions regarding care coordination and program inquiries.
• Generates referrals to providers, community-based resources, and other services to aid in goal achievement and health outcomes.
• Liaises between doctors, social workers, discharge planners, and community-based providers to coordinate care.
• Collaborates with the multi-disciplinary health care team to ensure care plans are person-centered.
• Documents encounters in appropriate systems per procedures.
• Overtime as necessary; additional duties as assigned.
