Job summary

The PSC acts as a key contact, delivering counseling, coaching, and service planning in the field, with duties spanning brokerage, group facilitation, and crisis intervention. The role emphasizes reusable engagement skills, data collection for funders, and warm handoffs to long-term providers.

Qualifications

• Lived personal experience with mental illness, homelessness, and/or co-occurring disorders.
• Must possess strong engagement skills.
• Strong organizational, detail orientated, and time management skills.
• Excellent verbal and written communication; able to juggle multiple priorities.
• California Driver's License and Own Vehicle.
• Must be able to gain clearance to visit jails and correctional facilities.
• Bilingual (Spanish, Mandarin, Cantonese, Vietnamese) preferred.

Responsibilities

• Provide outreach, engagement, and rapport building to IHOT participants.
• Use recovery stories to instill hope and support and inspire others.
• Provide direct services including linkage and referral to meet immediate needs and crisis counseling.
• Utilize CSTI, Stage-based Treatment, Motivational Interviewing for field support.
• Accompany participants to appointments and services.
• Provide community support groups and activities to build community.
• Provide coaching and role modeling to participants in the program.
• Link to long-term mental health providers and maintain case notes.
• Collect data for reports to funders and partners; bill services as required.
