VitalCaring Group is seeking a Field-Based Home Health RN to lead patient care in home settings, coordinating plans of care and driving outcomes from admission to discharge.

You’ll perform comprehensive in-home assessments, develop individualized care plans with physicians, and deliver skilled nursing care while educating families to support independence. A strong clinical judgment and autonomy are essential in this role.

Formación

• Active RN license (or compact eligibility) and CPR certification.
• Graduate of an accredited nursing program.
• Minimum 1 year RN experience (Home Health, Med-Surg, ICU, or Acute Care preferred).
• Strong clinical judgment and ability to think independently.
• Excellent communication skills and patient-centered approach.
• Proficiency with EMR systems.
• Valid driver’s license and reliable transportation.

Responsabilidades

• Conduct comprehensive in-home assessments to evaluate clinical status, risks, and goals of care.
• Develop and manage individualized care plans in collaboration with physicians and care teams.
• Deliver skilled nursing care, including medication management, treatments, and interventions.
• Serve as the case manager, coordinating care across therapists, aides, and physicians.
• Monitor patient progress and make clinical decisions based on changing conditions.
• Educate patients and families to support disease management and independence.
• Coordinate care transitions and discharge planning to reduce readmissions.
• Maintain timely, accurate documentation in the EMR.
• Supervise and support Home Health Aides as needed.
• Participate in on-call rotation as appropriate.
