Job summary

Rosaires Care is seeking a Primary Care Nurse to provide in-home, field-based RN services for adults and geriatric patients. You will assess, plan, and coordinate individualized care plans, manage medications, and deliver skilled nursing interventions in patients' homes.

Collaborating with physicians, therapists, and social workers, you will support safe transitions of care and coordinate treatment across a community-based service area.

Qualifications

• Current RN license.
• Strong clinical assessment and critical thinking skills.
• Ability to work independently in the home setting.
• Effective communication and collaboration with care teams.
• Compassionate, patient-centered approach.

Responsibilities

• Perform clinical assessments and ongoing patient monitoring.
• Create, coordinate, and adjust individualized care plans.
• Administer, reconcile, and ensure safe medication management.
• Provide skilled nursing interventions and patient education.
• Coordinate care with interdisciplinary teams to improve outcomes.
• Support safe transitions and continuity of care.
