Modern Machining Solutions Inc in Dallas seeks an experienced Home Health RN Case Manager for field-based care across Dallas/Fort Worth and surrounding counties. The role requires active Texas RN license and strong home health case management background.

You will perform skilled nursing visits, develop plans of care with physicians, complete OASIS documentation, and educate patients and families while traveling locally with reliable transportation.

Qualifications

• Active Texas RN license in good standing.
• Recent Home Health RN Case Management experience required.
• Strong understanding of Medicare home health, OASIS, plan-of-care management, and field-based nursing.
• Ability to work independently while maintaining strong communication with the clinical team.
• Strong clinical judgment, organization, documentation, and time-management skills.
• Valid driver's license, reliable transportation, and ability to travel locally throughout Fort Worth / Tarrant County.
• EMR experience preferred.

Responsabilités

• Manage assigned home health patient caseload in the field.
• Perform skilled nursing visits, assessments, evaluations, recertifications, resumptions of care, and discharges.
• Develop, update, and coordinate individualized plans of care in collaboration with physicians, patients, families, and the interdisciplinary care team.
• Complete accurate, timely clinical documentation, including OASIS and visit notes.
• Monitor patient progress, identify changes in condition, and communicate updates appropriately.
• Provide patient and caregiver education related to disease management, medications, safety, and plan-of-care goals.
• Coordinate care with therapy, social work, aides, physicians, and office/clinical leadership.
• Maintain compliance with home health regulatory, documentation, and quality standards.
