Job summary

Guardian Angel Home Care is seeking a Home Health Care Marketer / Liaison to drive field-based healthcare sales across hospitals, physicians, SNFs and assisted living facilities. This boots-on-the-ground role requires visiting referral sources and building face-to-face relationships daily.

You will educate staff on our Medicare-certified home health services, coordinate patient transitions, and track referrals in the CRM.

Qualifications

• Experience in healthcare sales, home health, hospice, pharma, SNF/ALF outreach, or similar.
• Strong communication and relationship-building skills.
• Ability to work independently and manage a territory.

Responsibilities

• Visiting referral sources (hospital, physician office, SNF, etc.).
• Introducing or maintaining relationships with clinicians and staff.
• Following up on patients and referrals.
• Educating staff on our Medicare-certified home health agency services.
• Coordinate patient transitions into home health services.
• Track all activity and referrals in CRM (Salesforce or similar).
