Job summary

Adaptive Home Health, LLC is seeking a Certified Occupational Therapy Assistant (COTA) to perform home health visits across Texas. This full-time, Monday–Friday role requires a Texas COTA license and the ability to travel within the assigned territory to deliver high-quality functional visits under the OT plan of care.

You will support ADLs/IADLs, safety/fall prevention, energy conservation, and adaptive equipment training while tracking progress and coordinating with the care team to ensure

Qualifications

• Active Texas COTA license in good standing.
• Ability to travel daily across assigned territory.
• Strong clinical judgment and independent field work within COTA scope.

Responsibilities

• Deliver skilled COTA visits in patients' homes under OT plan of care.
• Perform ADLs/IADLs, safety/fall prevention, energy conservation, UE function, AD equipment training.
• Track progress and communicate updates to supervising OT.
• Coordinate with nursing and broader care team regarding changes in condition.
