Job summary

Sanrose Home Health in Palm Desert, CA is seeking a Certified Occupational Therapy Assistant contracted to support OT services and ensure adherence to the Service Agreement. You will evaluate patients, assist with therapy plans, document progress, and coordinate with the supervising OT and care team to deliver effective home health services.

Qualifications

• Graduate of an occupational therapy assistant curriculum accredited by the American Occupational Therapy Association.
• Certified with the National Certification Examination of the American Occupational Therapy Association.
• Licensed as an occupational therapy assistant by the California Board of Occupational Therapy.
• A minimum of one (1) year of clinical experience as a certified occupational therapy assistant.
• Demonstrates good verbal and written communication, and organization skills.
• Possesses and maintains current CPR certification.
• Must possess a valid California Driver’s License with automobile that is in good working order and insured in accordance with the organization requirements.

Responsibilities

• Participates in the ongoing evaluation of patient’s functional status as delegated by the registered occupational therapist.
• Participates in teaching new skills or retraining patients in daily activities as needed.
• Maintains appropriate clinical records and reports to the registered occupational therapist any changes in the patient’s condition.
• Submits clinical documentation in accordance with Organization policy.
• Follows treatment programs and documents patient progress.
• Maintains contact with other personnel involved in the patient’s care to promote coordinated, efficient care.
• Attends and contributes to inservices and case conferences as required.
• Identifies patient and family needs for other home care services and assists with referrals.
• Participates in instructing patient’s family/caregiver and other Organization health care personnel in patient’s treatment regime.
• Is supervised by the registered occupational therapist no less than every thirty days; documents patient condition and outcomes.
