Job summary

Adaptive Home Health is seeking a Certified Occupational Therapy Assistant (COTA) for field-based home visits across Texas. This full-time role under OT supervision delivers ADLs/IADLs, safety training, and UE function work in patients' homes, with scheduling supported by technology.

Schedule: Monday–Friday. Compensation is productivity-based: $58 per point, 30 points per week. Must have Texas COTA license, valid driver's license, and ability to travel daily. Home health experience is a plus.

Qualifications

• Active Texas COTA license in good standing.
• Strong clinical judgment and ability to work independently in the field under OT supervision.
• Strong communication and documentation skills.
• Current driver's license and daily travel across assigned territory.
• Nice-to-have: prior home health experience.

Responsibilities

• Deliver skilled COTA visits in the home under the supervising OT's plan of care.
• Execute functional treatment plans: ADLs/IADLs, safety/fall prevention, energy conservation, UE function, adaptive equipment training.
• Track objective functional progress and communicate updates/concerns to the supervising OT.
• Coordinate with nursing and the broader care team; escalate changes in condition and safety concerns.
