Adaptive Home Health is building a higher-acuity, patient-centered home health model across Texas. We are seeking a Certified Occupational Therapy Assistant (COTA) for in-home visits in a field-based role with full-time hours and a Monday-Friday schedule, plus occasional weekends.

Compensation is $58 per point with an annual range of $80,000–$100,000. You will perform in-home OT evaluations, develop plans of care focused on ADLs/IADLs and safety, recommend adaptive equipment, and coordinate with

Competenze

Must-have Texas OT license (in good standing).
Strong clinical judgment and comfort working independently in the field.
Comfort escalating changes in condition to RN/MD and documenting rationale clearly.
Strong communication and documentation.
Reliable follow-through on orders, frequencies, and patient goals.
Current driver's license and comfort with daily travel across assigned territory.

Mansioni

Complete in-home OT evaluations and deliver skilled OT visits.
Build and execute a clear plan of care focused on ADLs/IADLs, safety/fall prevention, and functional independence.
Recommend and train on adaptive equipment and home modifications as needed.
Coordinate with nursing and the broader care team when condition or needs change.
Document promptly and thoroughly in Homecare Homebase (HCHB) - defensible, efficient, and consistent.
